If you were transported by the Village of Sussex Fire Department after December 16, 2008, you will receive an invoice from Lifequest Services. If you have any questions or concerns regarding the invoice or need to provide your insurance information, please call 1-800-786-4911 and Lifequest will assist you.
Prior to December 17, 2008 the Village of Sussex contracted with DAB (Diversified Ambulance Billing) to handle all of its Ambulance Invoicing and insurance claim filing.
If you were transported by the Village of Sussex Fire Department prior to December 17, 2008, you would have received an invoice which showed a return address of:
